Duties and Responsibilities Conducting initial evaluations on the client requirements and issues Developing health care plans tailored to each client's needs and in line with the digital program Providing guidelines to reduce or eliminate unhealthy behaviours. Working with other health care providers like OB/GYNs, fertility experts and other medical auxiliary staff Supporting clients in their recovery or rehabilitation. Resolving patient queries related to program modules, providing guidance as the patient navigates through the digital program Helping patients navigate and connect with healthcare providers, setting up patient appointments with health care providers
